Mathema Contest - Parental Consent

This form is used to collect verifiable parental consent for children under 13 who wish to participate.

Why we need this: In compliance with COPPA (Children's Online Privacy Protection Act), we require verifiable parental consent before collecting any personal information from children under 13. View our Children's Privacy Policy for more information.
